Children's Farm Home located in Corvallis, Parry Center for Children and Waverly Children's Home in Portland are managed by Trillium Family Services, Inc. a non-profit organization whose purpose is to provide advocacy and resources to families and engage them in the process of assisting the child in the development of necessary values and skills to become a positive, functioning member of society.  Our vision is to provide a continuum of comprehensive mental health services, promoting growth and learning in a safe environment for youth and families.

Program Description

 

Children's Farm Home offers the following services:

 

Assessment and Stabilization Program: High-acuity 90-day program offering intensive services including psychiatric evaluation and medication management, psychological evaluation, nursing services, individual, group and family therapy and on-site school.  Referrals to this program for children in DHS are directed to the DHS Residential Consultant.

 

JCAHO Psychiatric Residential Treatment Programs: Services include psychiatric evaluation and medication management, psychological evaluation, nursing services, individual, group and family therapy and on-site special education school.  Common problems referred include ADHD, mood disorders, anxiety disorder, thought disorder, psychosis and complicated diagnostic/clinical treatment issues.  Referrals are accepted from any source for these JCAHO programs.

 

JCAHO CISCO Program: Children’s Farm Home specialized program was implemented in May 1999 to serve 22 clients from twelve ABHA, BCN, and GOBHI counties.  ABHA counties include Benton, Deschutes, Jefferson, Crook and Lincoln.  BCN counties include Marion, Polk, Yamhill, Linn and Tillamook.  GOBHI includes Clatsop and Columbia.  Services range from JCAHO residential programs to in-home services.

Hawthorne Transition Center: Services include psychiatric consultation and medication management, nursing services, individual, group and family therapy and access to specialized community school programs.  Services are tailored for children completing residential programs and moving to a lower level of care.

 

BRS Therapeutic Foster Care Programs: Comprehensive services provided by professionals in specially trained foster homes.  Psychiatric consultation, individual and family therapy, and case-management services are available.  Common referrals include children whose behavioral and educational needs can be met in the public school system.  Three beds in the CFH program are designated for DHS Borderline IQ clients.

Program Description

 

Psychiatric Residential provides treatment for boys and girls ages 5 to 12 years old.  Psychiatric Residential treatment encompasses many elements including:  individual skills training; individual therapy (typically occurring one time per week or more as clinically indicated); group therapy; family therapy; multifamily support groups; horticulture therapy; expressive therapies (including music, art and dance); ancillary services are sought as indicated by the unique needs of the client.

 

Secure Children’s Inpatient Program (SCIP) provides the highest quality of mental health care to the most severely affected emotionally and psychologically at risk youth in our community through access to psychiatric, medical, psychological, nursing, and nutrition evaluations and services offered through the various treatment programs at Trillium Family Services.  A unique objective of the SCIP program will be the focus on returning youth to a less restrictive level of care.

Program Description

 

Waverly Children's Home provides the following services:

 

Residential Treatment serves emotionally disturbed boys and girls ages 6 to 14 years old.  These children require the structured, stable and nurturing environment that Waverly offers.  Our professional treatment staff helps children learn to build trusting relationships, improve daily living skills and develop a more positive self-image.  The goal of residential treatment is to give these children and their families the tools with which to form healthy relationships and to have them living together again as soon as possible.

 

School is an important part of every child's life at Waverly.  An individual Education Plan is developed for each child by Portland Public School’s special education teachers.  Parents and treatment staff are included in the development of this plan.  Educational personnel also provide individual transition plans for children returning to community schools.  Transition classrooms are located in two local public schools.

 

Therapeutic School and Day Treatment Program Services provides intensive daily treatment to boys and girls ages 3 to 14 and serves children with developmental delays, learning disorders, emotional disturbances, and/or behavioral difficulties, which preclude treatment in a less restrictive community setting.  These children have problems that keep them from being a successful participant in their school and community.  Children attend school in a special education classroom while participating in their own individualized treatment program.  Children live at home and share with their families and Waverly the goal of returning to their neighborhood school as soon as possible.

 

Outpatient Mental Health provides individual, group, couple and family therapy, with a primary focus on children’s mental health treatment and parenting concerns.  The primary diagnoses addressed include attention deficit hyperactivity disorder, anxiety, depression, multiple life challenges, learning problems and trauma experiences.

Supervised Visitation offers a safe, neutral meeting place for children and non-custodial parents, in the presence of a trained professional, when custodial and non-custodial parents are involved in visitation disputes.  This program helps to assure that parent-child relationships can continue when there is a perceived threat to the child's safety.  Non-custodial parents are taught parenting skills and child development.

 

Therapeutic Foster Care Program serves 20 clients, ages 6 to 17, and are specially trained foster homes with intensive case-management and therapy services drawing on resources provided by Trillium Family Services including psychiatric support.  Parry Care is another Therapeutic Foster Care Program, which serves six DHS clients in Multnomah County.

 

School-Based Support Program is a community based outreach program to children struggling to maintain in their home school communities in Multnomah County.  Diversion to Day Treatment, consultation and support to educational staff.  Wrap around in home services to families in the community.

·                                Behavioral Intervention (BIC) Program:  provides assessment and stabilization services to at-risk youth ages 11-14 years who have had difficulties in Supported Classrooms.

·                      Safety Achieved from Effective Prevention Teaming (SAFEPT) Program: provides support to the school community by identifying youth at-risk for violent behavior or mental health problems. 

·            Chrysalis and Powerful Feelings Program: provide psycho-educational group experience for high school and middle school youth who have experienced abuse.

 

Community Integrated Support for Children in Oregon (CISCO) Program is designed to provide individualized psychiatric treatment in a variety of settings and with flexible services to best meet the child’s clinical needs. In addition to our Trillium Family Services staff, family members, caseworkers, school staff and other key community supporters work together as a team to guide the course of treatment.  A unique array of services is available to provide the best fit for the child, family and community needs.
